RESUMO

Objective To evaluate the clinical safety and efficacy of focused ultrasound body contouring apparatus in abdominal fat reduction and body contouring. Methods Forty healthy subjeets were equally randomized into treatment group and control group, and received two treatments with the JCS-01 device (3H Medical Technology Co,Ltd,Beijing,China) in the abdominal region at one-week interval.During each treatment, the subject was placed on the treatment bed and the region of interest was spread with an acoustic coupling medium. Then the ultrasonic transducer was fixed, and the operator controlled the computerized system to move the bed in a designed regon. The control group would not receive any energy therapy during treatment time.Subjects were followed for 28 days after the last treatment (day 35).Abdominal circumference, regional photos and safety parameters were recorded at the time instantly before and after treatment and days 14 and 35.Subject satisfaction survey was conducted at day 35.Results One subject in control group was loss of follow-up.No local skin reactions, such as erythema edema or papules, and no changes with clinical significance in laboratory examinations occurred in all other 39 subjects. The proportion of subjects with over 0.5 cm abdominal circumference reduction in the treatment group was significantly higher than the control group at all time points (P<0.05).Peak abdominal circumference reduction was on day 35.The abdominal circumferences of 19/20 subjects in the treatment group were reduced by 0.83-6.33 cm,and the mean abdominal circumference was reduced by 3.09 cm on day 35.The total effective rate was 95 %,and the subject satisfactory rate was 75 %.Conclusions The focused ultrasound treatments for body contouring is safe and tolerable and also significantly reduces abdominal circumference.

RESUMO

Proteus syndrome is a rare complex disorder characterized by asymmetric and disproportionate overgrowth of multiple tissues, presence of cerebriform connective tissue nevi and epidermal nevi, vascular malformation and abnormality of fat tissues. In 2006,a patient with typical manifestation of Proteus syndrome was admitted to our hospital.By minimally invasive plastic surgery,we achieved good therapeutic results.

RESUMO

Objective To evaluate the effect of intratumor injection of Pingyangmycin for the treatment of superficial hemangioma and venous malformation. Methods From May 2005 to March 2008,42 patients with superficial hemangioma or venous malformation, including 17 men and 25 women(aged 3 months to 51 years)were treated in our hospital by injecting pingyangmycin intratumorally.Among the patients,18 had hemangioma,20 had venous malformation, and 4 had the both. The injection consisted of a mixture of Pingyangmycin(8 mg),2% lidocaine(2 ml),normal saline(2 ml),and dexamethasone(2.5 mg).The volume of the tumor was evaluated before the injection. And then the mixture was injected intratumorally at a density of 0.5 mg/cm2(0.3 mg/cm2 for the patients with ulcers in the skin or mouth). For the patients who had residual tumor after the first injection,a second treatment was performed in 10 days to 3 weeks or longer. The maximum dosage of Pingyangmycin for a single injection was 8 mg,and the maximum total dosage was 70 mg.Results One year after the treatment,85.5%(47/55)of the patients were cured, and 12.7%(7/55)were improved. The total effective rate was 98.2%(54/55).Conclusions Injection of Pingyangmycin is an effective and safe method for superficial hemangioma and venous malformation with low blood flow. The maximum injection density is 0.5mg/cm2.

RESUMO

OBJECTIVETo observe and analyze the pathohistological characteristics of capsules which formed around the mammary prosthesis with different contents. And to provide the selective basis for ideal and safe prosthesis in clinical practice.

METHODS20 specimen of the capsules were taken from 20 cases who receive the operation of prothesis removal for different reasons. HE, Masson and Mallory staining were used to analyse the tissue structure and characteristics under the light microscope.

RESULTSThe common structure including the collagen fibers accumulation, inflammatory cells infiltration and the capillary hyperplasia were found in all specimen. A layer of squamous epithelium-like cell was detected in some specimen. The specific characteristics were also found in different capsules formed around different prosthesis. In the capsules around vegetable oil prosthesis, there was excessive collagen fiber accumulation, and the capsules were much thicker. In the PVP (polyvinylpyrolidone) prosthesis capsules, there was severe inflammatory cell infiltration, and the number of eosinophilic granulocyte increased obviously. In the silicone gel and saline prosthesis capsule, the collagen fibers were well-arranged and the inflammatory cells were much less. Synovial metaplasia was detected in two cases.

CONCLUSION1. The capsules form around the prosthesis in all cases after mammary augmentation. 2. There will be synovial metaplasis in some cases, for vegetable oil prosthesis, the collagen over-accumulated which lead the capsules become thicker and harder. So it is not a kind of ideal mammary prothesis. 4. The severe infiltration of the inflammatory cells especially the large quantity of eosinophilic granulocyte indicate the possibility of the delayed hypersensitive reaction mediated by eosinophilic granulocyte. Cautious attitude should be taken during application.

RESUMO

Objective To investigate the expression of nestin in keloid and hypertrophic scarring. Methods Samples were obtained from patients with normal skin(n=8),flat scars(n=7),keloids(n=8) and hypertrophic scars(n=8),respectively.Immunohistochemistry staining was used to measure the nestin expression in above samples.Nestin positive cells were counted to evaluate the expression levels. Results ①Nestin expression was detected in basal cells, prickle cells, sweat glands, hair follicles, sebaceous glands, and vascular endothelial cells in 8 samples of normal skin, and in fibroblasts in 7 of them.Nestin was also positively expressed in abovementioned cells or skin appendages in samples of flat scars, keloids and hypertrophic scars.②Nestin positive cell counts(epidermic cells, fibroblasts, and vascular endothelial cells) in keloids and hypertrophic scars were significantly higher than those in normal skin and flat scars(P0.05).Conclusions The increased expression of nestin in epidermic cells, fibroblasts, and vascular endothelial cells in keloid and hypertrophic scarring supports a possible role of nestin as fibrosis inducing factors in keloids and hypertrophic scars.

RESUMO

Objective To improve the implatation of silicone prosthesis for rhinoplasty. Methods Forty one nasal specimens of cadavers were studied with anatomical and histological methods. And 213 cases of rhinoplasty via the retrofasial space (RFS) of dorsum nasi were performed. Results The upper extremity of RFS looked like bifurcation shape. Such surgery was performed for implatation by silicone prosthesis with V shape notch, and the rhinoplasty was successful with satisfactory effects. Conclusion Improved implatation of silicone prosthesis is an effective method for rhinoplasty.
